Understanding Activities of Daily Living (ADLs)

ADLs are routine activities that people perform daily without assistance. They are typically categorized into two groups: Basic Activities of Daily Living (BADLs) and Instrumental Activities of Daily Living (IADLs). ​

Basic Activities of Daily Living (BADLs)

These are fundamental self-care tasks necessary for basic functioning:​

  • Bathing and Showering: Maintaining personal hygiene through regular washing.​
  • Personal Hygiene and Grooming: Activities such as brushing teeth, combing hair, and nail care.​
  • Dressing: Selecting appropriate clothing and dressing oneself.
  • Toilet Hygiene: Getting to and from the toilet, cleaning oneself, and managing continence.​
  • Functional Mobility: Moving from one place to another while performing activities and is useful for people with varying physical abilities who can still move around independently.​
  • Self-Feeding: The act of eating by oneself, excluding meal preparation.​

Instrumental Activities of Daily Living (IADLs)

IADLs are more complex skills that allow a person to live independently within a community:

  • Cleaning and Maintaining the House: Tasks like tidying up, laundry, and basic home maintenance.​
  • Managing Money: Handling finances, budgeting, and paying bills.​
  • Moving Within the Community: Using transportation, whether driving or navigating public transit.​
  • Preparing Meals: Planning and cooking nutritious meals.​
  • Shopping for Groceries and Necessities: Purchasing food and other essential items.​
  • Taking Prescribed Medications: Managing and administering one’s medications correctly.​
  • Using Communication Devices: Operating telephones or other forms of communication to stay connected.​

The Role of Occupational Therapy in Promoting Independence

Occupational therapists play an important role in assisting seniors in maintaining their independence by focusing on both BADLs and IADLs. These professionals  work with older adults to help them participate in meaningful activities and live fulfilling lives. 

Occupational therapists perform falls risk assessments, evaluate clients’ functioning in their homes, and recommend specific home modifications to enable independence. 

For instance, when addressing low vision, occupational therapists might modify how daily tasks are performed and may recommend adjustments to a person’s environment. SImilarly, when  working with people with Alzheimer’s disease, they focus on maintaining quality of life and ensuring safety.

Assessing Independence in ADLs

Evaluating a senior’s ability to perform ADLs is important in determining the level of care or support they might require. The Functional Independence Measure (FIM) is one such tool. It uses a 0 to 7 scale to evaluate different ADLs based on the level of assistance required. A score of 7 indicates that the person is independent, while a score of 0 signifies that the person cannot perform the activity without assistance.

Strategies to Support Senior Independence in ADLs

Maintaining and enhancing a senior’s ability to perform ADLs involves a combination of environmental modifications, assistive technologies, and supportive services:​

  • Home Modifications: Making adjustments to the living environment can significantly impact a senior’s ability to perform ADLs safely. Simple changes like installing grab bars in bathrooms, ensuring adequate lighting, and removing tripping hazards can reduce the risk of falls and facilitate independence. Occupational therapists can assess clients’ functioning in their homes and recommend specific home modifications to enable independence. 
  • Assistive Devices: Various tools and gadgets are designed to assist seniors in performing daily tasks. For example, pill organizers can help manage medications, while reachers or grabbers can aid in picking up objects without bending. These devices can compensate for physical limitations and promote self-reliance.​
  • Community Support Services: Engaging with local senior centers or community organizations can provide access to resources and programs tailored to assist with ADLs. For instance, meal delivery services help to provide proper nutrition, while transportation programs facilitate mobility within the community. Participating in social activities also promotes mental and emotional well-being.​
  • Technology Solutions: The integration of the Internet of Things (IoT) in smart homes provides a remote monitoring system for keeping track of your loved one’s ADLs. This will help them to live on their own, knowing that a feedback alarm will be sent to their caregivers immediately in case of an emergency.
